I remember Mike Groom was trying to get custom boots made ages ago (very small). Long story, but I think he ended up with all the parts of a pair of boots just not sewn together, & then got a cobbler in Bris to put them together. Not sure how successful it was. Beefy might be able to get in touch with Mike.

another option might be to get orthotic style insoles made with closed cell foam inserts to push your foot back (ie foam at the toes). This might be the easiest option.

strap-on crampons should be ok for ice & mountaineering routes. Not sure how well they'd work for harder mixed routes. how hard do you plan on climbing?

I'm pretty sure the smallest Scarpa ski touring boots are 23 (mondo point).

PS: comp boots (ie fruit boots) suck, they are far too specialised for general winter climbing, & not very warm.
