After more thought on the meaning of my reply, I think I can simplify my essay -

There is complete onus on gym operaters to instruct to a suitable competency on safe, reliable equipment and structure. That is; the gym is responsible to ensure the walls are sound and not going to fall on anyone - harnesses, ropes etc. are at an acceptable level of safety - belay systems are up to standard - that all approved patrons should be able to competently belay on - and that they supply a professional level of instruction and supervision to all patrons.

The onus is then on the patrons to follow the rules and instruction, and use common-sense. Unless a gym enforces one on one/two supervision, or supply the belayers, you will never be able to completely ensure total supervision. Then people would have a right to complain about price increases.
