Ouch, I know how you feel!
I had the same problem a while ago with my Miuras - see previous thread. Are they unlined? If so, the 'bucket of water/walking in them' trick will almost certainly stretch the upper (but not the sole). If you want the rubber stretched you'll have to get them done at the Araps Mtn Shop or someone similar (does big John stretch shoes?). Dunno what effect this has on the integrity of the shoe. I almost had this done but chickened out and so far I'm just dealing with it and hoping my pain tolerance increases. So far it hasn't.
