Thanks hairy1 http://www.ntnews.com.au/article/2010/11/15/193971_ntnews.html ORIGINAL POST; Alice Springs abseiling fatality - Sat 13th Nov Early reports suggest that the abseiler ended up upside-down and falling out of their harness. (I assume this could only occur if the waist belt was loose enough to go over hips - or not done up at all) http://news.ninemsn.com.au/national/8140305/man-dies-in-abseiling-fall-from-bridge Condolences to family and friends of those involved.
