I think one of the major causes of Gri Gri failure is people 'thumbing' the device to pay out rope to allow a leader to clip. If the leader falls suddenly often a hand is still holding the device, not allowing the cam to engage. There really is no reason to hold the device at all when belaying (unless lowering someone), just hold the active and dead ends of the rope to pay in and out of the GriGri. I've have always used a method where I would keep around 6 feet of slack in the system at all times (unless obviously there is the potential for groundfall). That way there is 3 foot doubled which seems is enough for most to reach and clip with. Then I never have the problem of the leader getting in flap, yanking for slack, the device locking up and the need to 'thumb' the device to pay out rope in a hurry. Also, as has been mentioned, a bit of slack makes for a softer catch.
