Often climbers abseil after doing a climb and are often tired and not concentrating (especially on long multipitch routes). While retreating of my first big wall route I almost killed myself three times and this was certainly attributed to me being incredibly tired and dehydrated, i) abseiling when I only put one end of a double rope through a belay device, ii) decided it was okay to abseil off one nut, iii) deciding it was okay to abseil of a single piton (which I was anchored to and my partner subsequently pulled out by hand!!!). All three time my partner noticed my error. I am now very aware of the fact that I am often tired when abseiling and double check everything and do a check with my partner aswell.
