A lever doesn't have to have the fulcrum in between the load and applied force. If you look at your elbow, the muscle (applied force) is just next to the joint, while the load is applied further along the same side at your wrist/hand, so applying a force out there has a much bigger mechanical advantage (longer lever arm) than what your muscle has. This kind of lever is designed so that the applied force only has to move a small distance to get a large movement out at the load end. That and otherwise, your bicep would have to be joined to your hand or something freaky = ) (sorry to get all boring and sciencey, I'm an engineer who's just on a lunch break)
