ok ill try to keep this one simple and short!!

Im a physiotherapist who specialises in the lumbo-sacral region

technically sciatica is caused by compression of the sciatic nerve between the layers of your piriformis (a muscle in your butt) as apposed to nerve root pain caused by compression of a nerve root at a given level caused by protusion of disc material.

Assuming here your issue is nerve root or at least discogenic pain (true sciatica isnt common) its important to establish why your having these attacks

posture yes im stereotypical plays a role as sustained forward bend such as slumped sitting is complete disaster as is repetitive lifting ect we all know this.

your right core stability is critical but is also generally very poorly taught by health professionals. You can do core abdominal to the cows come home but if your pelvic floor is SH** than it wont do jack.

Fit balls are great in the right hands as is pilates but both are impossible to teach yourself.

I could go on here all day, yes i enjoy what i do for a living! but i will say
1: get a diagnosis, a good physio, osteopath ect should have a good idea before you spend 300 on an MRI

2; see someone who has some sort of interest not only in fixing you but keeping you fixed ie your right the core is critical

3. depending on where you are in the world diagnosic real time ultrasound which shows you, your core muscle as you learn to work them is the best way to learn about your core. we are all very different. google real time ultrasound

4. as a side suggestion if you have had any kind of trauma eg, hard fall, ground fall car accident ect your hip/back (SIJ) joints could be playing a role . i see this in people who have had lots of treatment and just dont ever get quite right.

5: find someone, anyone that you trust to treat you. they will make you sore during treatment and they will give you exercises that are difficult!

good luck i hope this is of some value.
