I wonder if this was the same 'setup in the wrong place', that lead to hitting the ledge previously?

'Lucky' has that place pretty well dialled due to having been there (often-ish) before...
If he was involved, then it's likely that there is more to this than first glance would reveal.

Very sad to hear this. After all he went through recovering from his last accident, to go like this, on something he was so familiar with... just terrible. It's all worth it right up until it isn't. Heavy condolences to family and friends, he was really one in a million.

Does anyone know what happened?
Having met the guy only in passing he seemed to live up to his personality in the media. I'm saddened to see this happen - for his family and friends but also for the climbing community.

It seems very hard to get the coroner's reports. Having looked for a few, I think there is no formal advice issued if there isn't a recommendation to make a case for the police (someone to be charged). Can anyone correct me?
I think there's more benefit from speculation and collation of available facts than getting the facts late or never.

I would assume those there did there best.
They will come forward or not as they wish. I feel sorry for them all.

Is that a new(ish) feature?
I wanted to read one once and was refused, it was not released 'for the families'.
