I'm not sure of the relevant dates and outcome, but there were serious issues with the Choucas harness and Blue Ice made some changes. Should come up with a Google search. I know it was mentioned on UKClimbing and also maybe on Dane's Coldthistle blog. I think the buckles were failing? Best to check.
